LTCC/WAIVER
PROGRAMS/ ALTERNATIVES TO
NURSING HOME
Todd County Public Health has information about our Long Term Care Consultation (LTCC) Service in Todd County. The LTCC is available to all residents of Todd County regardless of age. There is no fee charged to the individual for the LTCC.
A LTCC involves a visit by a public health nurse and/or licensed social worker to the individual’s home requesting this service. Various health care options for home health services and community resources will be discussed during the LTCC to match the services and resources to the needs and preferences of the individual.
There are waiver programs available within Minnesota. These programs provide funding for home health care services and community supports to help prevent, or delay the need for nursing home placement for seniors and/ or individuals with disabilities. To be eligible for a waiver program, the individual must be at risk for nursing home level of care and meet financial eligibility requirements. The LTCC team can determine and individual’s eligibility to obtain funding through these waiver programs for home health care services and community resources.
